2. Parent/Guardian Testimony (included in Section VI of the Application)


3. Application Fee of $100 (non-refundable)


4. Pastoral Letter of Reference (click here to view/print)


5. A signed copy of the Honor Code (a parent/guardian may sign for grades K-2nd)


6. A signed copy of the Statement of Faith (included on page 6 of Application)


7. A copy of student's previous school's records (grades and standardized test scores) or homeschooler's annual testing and/or evaluation

After the interviews, if a student is accepted, the parents will receive an Acceptance Letter, and the Transfer of Records form will be sent to the previous school attended. In order for the admission process to be final, the Academy must receive the following items:

• Enrollment Contract for 2008-09 with $200 Enrollment Fee
• Copy of Birth Certificate
• Proof of Medical Insurance
• Medical Information Form
• Florida School Entry Health Exam (view/print)
• Florida School Entry Health Information for Parents
• Florida Certificate of Immunization (blue form) - or proper exemption is required under state law before the child is allowed to attend school.
